- Have a Body Mass Index (BMI) of 40 kg/m² or more (or 37.5 kg/m² for people from minority ethnic family backgrounds)

- Have at least four of the following long-term conditions:

1.    Type 2 diabetes

2.    Hypertension (high blood pressure)

3.    Dyslipidaemia

4.    Established cardiovascular disease

5.    Obstructive sleep apnoea

Before requesting an appointment at your GP Practice, please consider the alternatives. The pharmacist at your local chemist is highly trained in offering a wide range of services and may be able to give you the help you need sooner.
